Né à Montpellier, il aurait vécu dans la deuxième moitié du XIVe siècle. Parti jeune pour un pèlerinage à Rome, il a soigné, tout le long du chemin les malades de la peste, grâce à ses capacités de guérisseur, l’épidémie sévissant alors partout en Europe.
La légende veut que, sur le chemin du retour, ayant lui-même contracté la peste et souffrant d'un énorme bubon à la cuisse, il se serait retiré dans un bois près de Plaisance, en Italie, avec son chien pour seule compagnie. Chaque jour, l'animal revenait avec un pain. Puis un ange le guérit miraculeusement. Il reprit sa route, mais fut fait prisonnier, pris pour un espion, et resta cinq ans au cachot à Voghera, près de Milan avant de mourir.
Son culte naît dès la fin du XIVe siècle, pour devenir, le saint antipesteux le plus vénéré en Europe ; sinon le seul.
Saint Jacques, dans une bien moindre mesure, était également invoqué en temps d'épidémies de peste. L'iconographie différencie cependant les deux saints en montrant saint Roch retroussant sa robe de pèlerin et découvrant son bubon, qu’un ange soigne. A ses pieds, le chien, un pain dans la gueule, le lui tend.
